Driffield Jiu-Jitsu Academy liability waiver/informed consent form
Assumption of risk and release of liability
I the participant specify below, have enrolled in a personalised health and fitness programme offered by Driffield Jiu-Jitsu Academy I recognise that the programme may involve strenuous physical activity including but not limited to muscular strength training, cardiovascular training, martial arts training, sparring and other physically strenuous physical activity.
I hear by affirm that I am in good enough physical condition to participate in this programme and do not suffer from any physical ailment that would prevent or limit me from participating in this programme.
I the participant specified below am aware of the subsequent risks of all aspects of athletic activities and physical training. These risks include but are not limited to: falls that can cause serious injury or death; injury or death that is due to negligence of myself, my training partner, or other people around me; injury or death related to improper use or failure of equipment; sprains and strains. I am aware that any of the aforementioned risks may result in serious injury or death to myself.
In consideration of my participation of this programme I hear by release Driffield Jiu-Jitsu Academy and its principles, agents, employees, volunteers from any claims, demands and causes of action as a result of my voluntary participation and enrolment.
I, the participant specified below, fully understand that I may injure myself as a result of my enrolment and participation in this programme and hear-by release Driffield Jiu-Jitsu Academy its agents from any liability now or in the future for conditions or injuries that I may obtain. These conditions may include but are not limited to, heart attacks, muscle pulls, muscle strains, muscle tears, broken bones, shin splints, ligament damage, traumas, anxiety, fears, injuries to the back, injuries to feet, or any other injuries that may occur including death.
Emergency medical services
I the participant specified below, give permission to Driffield Jiu-Jitsu Academy and trainers to seek emergency medical services for me if I am to become injured or ill with the understanding that I am responsible for any expense incurred.
If I am signing on the behalf of a minor, I also give full permission for any person connected with Driffield Jiu-Jitsu Academy to administer first aid if necessary In case of serious injury or illness.
Indemnity
I, the participant specified below, accept financial responsibility that I may cause to either myself or any other participant due to my negligence.
I further agree to indemnify and hold harmless Driffield Jiu-Jitsu Academy there principles, agents, employees, Voluntary assistants and coaches from liability for the injury or death of any person(s) and damage to property that may result from my neglect or intentional act or omission whilst participating in activities offered by Driffield Jiu-Jitsu Academy This not limited to parks, recreational areas, playgrounds, areas adjacent and/or any area selected by Driffield Jiu-Jitsu Academy.
Photography and video
I, the participant specified below, acknowledge that I may be photographed or video taped during training. I hearby consent to the use of these photographs or videos to be used without compensation on the Driffield Jiu-Jitsu Academy or in any other advertising/promotional material produced byDriffield Jiu-Jitsu Academy.
Important note
I am aware that this agreement is ongoing and will apply to all future occasions I participate in athletic activities with or at the direction of Driffield Jiu-Jitsu Academy.
This agreement shall be binding upon me, my successors, representatives, heirs, executors, assigns or transferee’s. If any portion of this agreement is held invalid, I agree that the remainder of this agreement shall remain in full legal force and effect.
I have read and understand the above statements. I understand that by signing it obligated me to indemnify the parties named for any injury, for liability or death of any person and damage to property caused by my negligence or intentional act or omission.
